Here, at Bluewater Area Family Health Team, our Mission is to provide outstanding primary health care. To ensure we are providing this support we’re launching a survey of our current patients to be able to better provide appropriate mental health supports. Over the next two weeks you will receive information from our office outlining an upcoming survey to gather information from our patients to provide the care you need and want. The survey is approximately 18 questions and will take less than 10 minutes to complete and all questions are anonymous and confidential. By participating, you will be entered in a draw to win one of ten $25 Jerry Rader’s gift cards! The survey can be completed online or using a paper copy provided in person at office reception. The survey will launch May 1st, 2023.
